Fourteen NHS trusts' maternity care to be investigated over 'failures'

Baroness Amos' investigation focuses on systemic maternity care failures over 15 years across 14 NHS trusts, aiming for national recommendations to improve safety and address inequalities.

  • On Monday the Department of Health and Social Care confirmed the Shrewsbury and Telford Hospital NHS Trust will be examined by Baroness Valerie Amos, who leads an independent probe into systemic maternity problems over 15 years.
  • Independent reviews have revealed a repeating pattern of failures across trusts, including SaTH's maternity services which were found to have failed women, children and families in earlier inquiries.
  • Selected trusts were chosen using CQC, MBRRACE-UK data and family feedback, with the government naming 14 NHS trusts for examination and interim recommendations due in December 2025.
  • He said: `Bereaved families have shown extraordinary courage in coming forward to help inform this rapid national investigation alongside Baroness Amos`, and SaTH's chief executive Jo Williams expressed support and willingness to share progress.
  • The review will run alongside a National Maternity and Neonatal Taskforce chaired by the Health and Social Care Secretary Wes Streeting, with Baroness Valerie Amos focusing on inequalities affecting Black and Asian women and marginalised groups.
