Inquiry launched into 'repeated' maternity failings at Leeds NHS trust

The inquiry follows reports of at least 56 potentially preventable baby deaths and highlights concerns over poor leadership and a blame culture, officials said.

  • Health Secretary Wes Streeting announced an independent inquiry into Leeds Teaching Hospitals NHS Trust's maternity units after meeting bereaved families who welcomed the investigation.
  • Earlier this year a BBC investigation found at least 56 baby deaths and two mothers at Leeds Teaching Hospitals NHS Trust may have been preventable, while the Care Quality Commission downgraded the trust to "inadequate" in June.
  • Maternity Family Support Group Leeds, which is 150-strong, and the BBC spoke to more than 70 families who say they were dismissed and blamed, and Amarjit Kaur said, `We know we are not alone, and that there's other families that have experienced what we have.`
  • The trust issued an apology and said it is already taking steps to improve maternity and neonatal services, while the Department of Health has not yet published the inquiry's terms of reference.
  • As part of a national review led by Baroness Valerie Amos, LTH is one of 14 trusts examined, and families want senior midwife Donna Ockenden to chair the Leeds inquiry for her experience.
