Investigation shows Warren County emergency alert issue was 'human-caused'

WARREN COUNTY, OHIO, JUL 29 – Training will be enhanced after a mapping error caused an emergency alert to reach multiple counties instead of only Deerfield Township, affecting tens of thousands of residents, officials said.
