Woman dies after falling into an uncovered maintenance hole in New York City
Con Edison says video suggests a heavy truck dislodged the cover about 12 minutes before the 56-year-old fell in, as investigators review the death.

- Con Edison spokesperson Elisabeth Ferrari said the company is "actively investigating" and stated, "Our thoughts are with the individual" after the woman was pronounced dead at Weill Cornell Medical Center.
