Oregon Wellness Network Leads National Team Selected as Winner in Federal Health at Home Challenge
The consortium will use a shared electronic medical record and artificial intelligence to coordinate care for more than 100,000 Medicare and Medicaid beneficiaries.
On Tuesday, October 6, 2026, the Oregon Wellness Network announced it is leading one of ten winning teams selected nationwide in the federal Health at Home Challenge, run by the U.S. Department of Health and Human Services.
Operating on a single shared AthenaOne electronic medical record—a first for community-based care at this scale—the eight-hub consortium integrates artificial intelligence to help care teams identify at-risk individuals earlier.
Together, the eight hubs aim to serve more than 100,000 Medicare and Medicaid beneficiaries with chronic conditions. Director Lavinia Goto of the Oregon Wellness Network called the integrated network "a new capability for this nation."
Services span five care pathways including hospital-to-home transitions, fall prevention, and depression support. Region IV Area Agency on Aging CEO Christene Vanlandingham said success means "helping more people spend more healthy days at home."
The team now advances to Phase 2 of the Challenge, running through July 2027, with up to five teams selected next summer to advance to the final scaling phase.
