- On July 6, 2025, numerous vigilante members from Kanam Local Government Area organized an unauthorised operation in Kukawa community, Plateau State.
- The vigilantes conducted an unauthorized operation without informing Operation Safe Haven forces, during which they reportedly clashed with local residents, ransacked multiple stores, and took possession of more than 20 motorcycles prior to the ambush.
- While moving toward Odare Forest, the team was attacked by armed bandits, leading to the deaths of eight vigilantes and leaving several others unaccounted for as search and rescue operations continue.
- Major Samson Zhakom, OPSH media officer, confirmed the incident while clarifying that some media reports contained inaccuracies, and noted that the bodies of those killed have been found and laid to rest.
- OPSH urged vigilante groups to coordinate with security forces before operations, warning unauthorised actions endanger lives and calling on residents to support security efforts.
