- After reports of a disabled semi, Trooper Cayton responded to a Kenworth semi tractor-trailer, whose driver said it struck debris and became disabled on State Route 11 northbound, south of U.S. 224.
- A Mack Granite truck driven by Ryan Rach rear-ended Cayton's cruiser, pushing it into the disabled tractor-trailer and striking the semi driver; Rach was uninjured.
- Trooper Nicholas P. Cayton was pronounced dead at the scene, and Nelson De Jesus Herrera Vasquez, the semi driver Cayton was assisting, was flown to a nearby hospital with serious injuries.
- With the scene still under investigation, the Ohio State Highway Patrol said more information will be released and Ohio Gov. Mike DeWine ordered flags at half-staff across Mahoning County and state buildings.
