3 Dead, 2 Children Hospitalized After Being Exposed to Carbon Monoxide in Vehicle Pulled over for Flat Tire

On Wednesday, July 15, 2026, five people were found unresponsive inside a Ford Explorer in Toledo, Ohio, after the vehicle pulled over with a flat tire; three died and two children remain in critical condition.
Toledo Fire Chief Allison Armstrong confirmed a "high spike of carbon monoxide" inside the SUV, with foul play ruled out while investigators determine how the gas entered the passenger compartment.
Armstrong recalled a "very similar" incident several years ago involving carbon monoxide seeping through floorboard holes, and the vehicle involved in Wednesday's tragedy was towed by police for further inspection.
Ford Explorers underwent a six-year federal investigation into exhaust odors; in 2023, the National Highway Traffic Safety Administration reviewed more than 6,500 consumer complaints but determined the SUVs did not require a recall.
That federal probe covered nearly 1.5 million Explorers from the 2011 to 2017 model years, though the specific model year of the vehicle involved in Wednesday's incident remains unknown.
