NTSB opens hearings on why UPS cargo plane engine fell off, causing deadly crash
Investigators are examining maintenance records and reporting gaps after records showed 10 prior MD-11 bearing flaws, only four of them reported to the FAA.
- On Tuesday, the National Transportation Safety Board opened a two-day investigative hearing in Washington regarding the November 4, 2025, crash of UPS Flight 2976. Newly released surveillance video shows the left engine separating during takeoff, killing 15 people.
- Investigators discovered the crash resulted from structural failure after a spherical bearing fractured in the pylon. Records revealed 10 prior flaws in similar MD-11 components, though Boeing documented the issues years ago and only four were reported to the Federal Aviation Administration .
- Boeing officials testified the pylon attachment was considered "failsafe," meaning if one structural lug failed, the other should support the load. The FAA countered that the part should have been classified as a Principal Structural Element, requiring stricter inspections.
- Families of victims attended the hearings seeking answers, with Attorney Bradley Cosgrove of Clifford Law Offices stating they deserve accountability. "These families are devastated and certainly deserve answers," Cosgrove said.
- While FedEx resumed MD-11 operations after the FAA approved Boeing's repair protocol on May 10, UPS announced plans to retire its fleet. The safety board will issue final recommendations later this year addressing the maintenance and reporting failures identified.

NTSB: Flaw that led to engine flying off UPS plane grew unnoticed under relaxed inspection schedule
By JOSH FUNK A UPS plane crash that killed 15 people last year might have been prevented if an original inspection schedule hadn’t been relaxed, but mechanics didn’t get a close look at the parts that should have kept an engine from flying off its wing because federal regulators allowed Boeing to recommend checking them less frequently, according to testimony Wednesday. The National Transportation Safety Board’s questions also drew out that Boei…
The NTSB, an independent U.S. government agency that investigates major transportation accidents, shared footage of a fatal plane crash on Tuesday. The footage shows the plane losing an engine.
