Mum and Daughter Found Dead in Their Home Months After 999 Call

RADFORD, NOTTINGHAM, JUL 21 – An emergency call requesting an ambulance was mistakenly closed without dispatch, resulting in no response and the deaths of a mother with sickle cell anaemia and her dependent daughter.

  • On Monday, July 21, 2025, Nottingham Coroners’ Court opened an inquest into the deaths of Alphonsine Djiako Leuga and Loraine Choulla, found dead at 85 Hartley Road, Radford, Nottingham, after their bodies were discovered in May 2024.
  • Pragmatically discharged to care for her daughter, she soon made an emergency call on February 2, 2024 at 1.04pm requesting help.
  • Nottingham Coroner’s Court heard that the call was closed as abandoned, and it was noted that EMAS’s safety chief, Susan Jevons, said the ambulance didn’t attend because the call handler closed it, despite the address being obtained.
  • While reflecting on the tragedy, Amanda Bewley warned that sending an ambulance might have been “the difference between life and death” for Loraine.
  • Over five days, the court will hear evidence until Friday, July 25, 2025, with formal conclusions due on that date.
