Senators Accuse Telcos of Triple Zero ‘Cover-up’ over System Failures
TPG identified hundreds of thousands of incompatible Samsung phones blocked from its network, with at least two deaths linked to failed Triple Zero calls on outdated software.
- On December 8, 2025, TPG Telecom said it was made aware that calls to Triple Zero may have been linked to someone who has died, Inaki Berroeta told a Senate inquiry.
- TPG says device compatibility problems and a 3G shutdown prevented some older Samsung handsets running incompatible software from connecting to Triple Zero.
- One affected customer later reached emergency services via the National Relay Service, about five minutes after initial failed calls at Wentworth Falls, NSW's Blue Mountains.
- TPG has provided information to the Australian Communications and Media Authority and says it will continue doing so, while executives from Nokia, Ericsson, Telstra, and Samsung face questions at the Senate hearings in Canberra.
- Telcos have faced scrutiny in recent months after the Optus outage, and Berroeta warned that many non-compliant phones are blocked, urging affected customers to update or replace devices.
