NTSB opens hearings on why UPS cargo plane engine fell off, causing deadly crash
Investigators are examining maintenance gaps and reporting failures after video showed the left engine separating from the jet during takeoff.
- On Tuesday, the National Transportation Safety Board opened a two-day hearing in Washington to examine the deadly UPS cargo plane crash in Louisville, Kentucky, that killed 15 people in November 2025.
- Surveillance video released during the hearings shows the left engine of UPS flight 2976 separating and flying over the wing, engulfed in flames, as the MD-11 accelerated down the runway at Muhammad Ali International Airport.
- Investigators found records of 10 previous flaws in engine mount parts on similar aircraft, with NTSB member Tom Chapman noting only four of the 10 were reported to the Federal Aviation Administration; Boeing determined they would not result in a safety-of-flight condition.
- UPS official David Springer testified that Boeing's service letters made the bearing problem 'sound almost benign' and failed to disclose collateral damage to engine lugs, saying UPS would have asked 'different questions over the years' if fully informed.
- While FedEx resumed MD-11 operations on May 10 after the FAA approved Boeing's repair plan, UPS plans to retire its entire fleet; the NTSB's final report likely will not arrive for more than a year as investigators examine every potential factor.
