Hull dad who took his own life 'let down by health services'

John Caulfield, of west Hull, was described as 'fit and healthy' But in March 2022 he began suffering from anxiety and his mental health deteriorated. He was referred to the mental health team by his GP and then a more urgent referral by a nurse a week later. In mid-April, after expressing suicidal thoughts he was sent to the Avondale assessment unit at Miranda House. He then took an overdose early in November and was once again referred to mental health services run by the Humber Teaching NHS Foundation Trust. But by November 10, he was discharged with no mental health support in place. An inquest into John's death was held on Monday and assistant coroner Sarah Middleton delivered her conclusion.
