Gloves left inside patients and accidental organ removal among 403 'never' mistakes in NHS last year
NHS data show 121 retained-object cases and 166 wrong-site surgeries, with one patient having an organ removed by mistake.
NHS England recorded 403 'never events' in the past year, matching the previous year's total of serious, preventable patient safety incidents.
Wrong-Site surgery dominated the incidents, accounting for 166 cases including 40 treatments administered to the incorrect body part and 17 procedures performed on the wrong patient.
Overall, 121 'never events' involved foreign objects left in patients, including 26 guide wires and 53 swabs; 50 incidents involved incorrect implants, notably 14 knee replacements and nine eye lenses.
Additional errors included 17 cases where medication was administered by the wrong route, with 15 involving oral drugs given intravenously, and 14 insulin overdoses from incorrect syringes.
An NHS spokesperson said staff work exceptionally hard to keep patients safe and that trusts must investigate each incident and take effective steps to learn and improve.
