Investigators Look Into 'Repeating Bell' Heard During Takeoff of UPS Cargo Plane that Crashed

The 34-year-old UPS plane had structural repairs completed after being grounded for over a month and flew about 70 hours before crashing, NTSB said.

  • On Nov. 4, the UPS MD-11 cargo plane crashed shortly after takeoff from Louisville's UPS Worldport, with airport footage capturing the left engine detaching, killing 13, including the pilots.
  • After being returned to service, Federal Aviation Administration records show the 34-year-old MD-11 aircraft was grounded from Sept. 3 to Oct. 18 for repairs addressing a crack in the center wing upper fuel tank and corrosion at San Antonio International Airport.
  • The cockpit voice recorder shows a repeating bell about 37 seconds after takeoff, which continued until impact, while flight data shows 210 mph at 100 feet before crash, investigators say.
  • On Nov. 7, law firms Whiteford Taylor & Preston and Peterson Law said they will represent several Kentucky families and businesses, with Masten Childers III calling it a "heartbreaking loss".
  • The NTSB said it will review every piece of maintenance from San Antonio through the flight and convene its CVR technical group to transcribe audio, while examining CCTV footage, flight data, and the MD-11 fleet's aging safety record.
