Swaffham Co-Op Undertaker Crushed to Death by Lift

Sally Blundell was crushed by a 200kg hydraulic lift while working alone, with investigations examining equipment faults and lone-working safety, officials said.

  • On December 1, 2023, Sally Blundell, 58, funeral administrator, was found underneath a scissor lift in the morgue at the East of England Co-op Funeral Services branch in Swaffham, Norfolk.
  • The coroner said a hydraulic hoist came down on her body, crushing her chest, and Norfolk area coroner Yvonne Blake recorded the cause as contusion and compression of the chest by an external object.
  • A colleague told the inquest that he arrived just after midday, after a family raised concerns about an 11am appointment, and CCTV showed two women in reception but no staff in sensitive areas.
  • The inquest with a jury continues after an initial police investigation, while the local authority investigation examines the trolley’s condition, lone-working arrangements, staff training, and the hourly lone-worker alarm at 10am that was manually closed.
  • HSE examination found faults in the 200kg hydraulic scissor lift mortuary trolley , identifying return-spring and control-handle faults and noting descent time and a height no greater than 5ft .
