Man killed by tiger may have had medical records breached by NHS staff
The trust said proactive audits found two possible unauthorized access attempts and reported the case to the Information Commissioner's Office.
Yorkshire Ambulance Service Trust launched an investigation into two staff members who potentially accessed William Chapman's medical records "without a legitimate reason," identified through proactive internal audits.
On Saturday, William Chapman, 24, died after scaling two fences at the "Land of the Tiger" enclosure near Doncaster; the inquest confirmed he was mauled to death.
An inquest at Doncaster Coroner's Court heard Chapman had a history of mental illness but was "highly functioning and independent with no care requirements," while his family expressed "a number of concerns" about his mental health services.
The trust reported the matter to the Information Commissioner's Office and stated it will keep Chapman's family informed, adding their "thoughts are with William Chapman's family at this distressing time."
Yorkshire Wildlife Park reopened on Friday with Altai, the Siberian tiger involved, confirmed "safe and well," and the park's animal care team prioritizing the tiger's "ongoing welfare at YWP.
