Hospital apologises for failings after mental health patient takes her own life

Peter Mullis had been taken to Burton's Queen's Hospital after he fainted. The 77-year-old, who was described as an "escape artist" by his son-in-law, managed to get out of the hospital. When outside, he climbed over a metal barrier that was only three feet high. Despite staff warning him of the dangers, Mr Mullis fell down the slope and hit his head on the concrete at the bottom. He was airlifted to another hospital but later died from a catastrophic head injury.
