Family Identifies 5 Victims, Including 4 Children, Found in SUV with Suspected Carbon Monoxide Poisoning

Officials said a high spike of carbon monoxide filled the SUV’s passenger compartment, and investigators are still determining how the gas entered.

  • On Wednesday, five people were found unresponsive inside a parked Ford Explorer in Toledo, Ohio, after the vehicle pulled over with a flat tire. Toledo Fire Chief Allison Armstrong confirmed extreme levels of carbon monoxide caused the tragedy.
  • Investigators confirmed the carbon monoxide level was five times the lethal limit inside the vehicle. The victims included 36-year-old grandmother Candace McAllister and four young children: Liberty, Storm, Legacy, and Stone.
  • Adriana Coleman, the children's mother, made the difficult decision Friday to remove her son Legacy from life support. Three other family members had already died earlier in the week.
  • Toledo Mayor Wade Kapszukiewicz stated foul play is not suspected, while officials continue investigating how the gas entered the cabin. The family established a fund for funeral expenses at any Fifth Third Bank location.
  • The federal government previously conducted a six-year investigation into exhaust odors in nearly 1.5 million Ford Explorers. Although a 2023 probe determined no recall was necessary, experts are currently examining how carbon monoxide entered this specific vehicle.
