Errors in Disability Cases for ‘Painful Scars’ Resulted in Overpayment: VA Watchdog
The audit said inconsistent scar evaluations led to 17,900 errors and $5.6 million in overpayments, while the VA pledged corrective reviews.
A new federal audit revealed nearly 18,000 errors in Department of Veterans Affairs disability claims for painful scars, resulting in at least $5.6 million in overpayments between June 1, 2024, and May 31, 2025.
Investigators found that claims processors often relied on subjective reports without objective evidence, while systems failed to flag 'pyramiding'—the improper practice of evaluating the same disability manifestations under multiple diagnostic codes.
Questioning the methodology, the VA noted auditors relied on a 130-case sample to project thousands of errors, emphasizing only seven instances resulted in improper payments, representing about 5.4 percent of the sample.
Veterans Benefits Administration committed to re-examining incorrect decisions and plans to release a revised Scars/Disfigurement Benefits Disability Questionnaire by Jan. 10, 2027, ensuring processors have necessary evidence.
Unless corrective actions are taken, the Inspector General warned that overpayments will continue to accrue. Veterans may face repayment requirements even if overpayments resulted from calculation errors, according to industry reports.
