'Catastrophic' Brake Failure Led to Icefield Rollover in Alberta that Killed 3 People
Judge Vaughn Myers said the Ice Explorer had worn parts and weak brakes, and called for stronger inspection and maintenance rules.
A fatality inquiry report released last week by Alberta Court of Justice Judge Vaughn Myers concluded that "catastrophic" brake failure and neglected repairs caused the 2020 Columbia Icefield tour bus rollover involving operator Pursuit.
The vehicle accelerated to nearly 50 km/h on a 40 per cent grade exceeding the manufacturer's maximum 36 per cent limit, as severe mechanical defects left it with insufficient braking power.
A previous 1,000-hour inspection by Wajax was undated and failed to identify the faulty system, even though a driver reported major braking issues five days before the crash.
Pursuit pleaded guilty in 2023 to two charges under the Alberta Occupational Health and Safety Act, incurring a $475,000 fine, and has since implemented safety changes including seatbelts and enhanced driver training.
Myers encouraged OHS and Alberta Transportation to establish standardized three-year inspection procedures for off-road tour vehicles, emphasizing the inquiry aimed to examine maintenance lapses and prevent similar tragedies.
