Deep Learning AI Model Identifies First-of-Its-Kind Biomarker of Chronic Stress

The Adrenal Volume Index, derived from chest CT scans of 2,842 adults, correlates with stress markers and predicts cardiovascular risks and mortality, researchers report.

  • On Nov. 25, 2025, Elena Ghotbi, M.D., and colleagues at Johns Hopkins University announced that a deep-learning AI model identified the first imaging biomarker of chronic stress, with findings to be presented next week at the Radiological Society of North America.

  • Applying the AI model to the cohort, the team analyzed 2,842 participants from the Multi-Ethnic Study of Atherosclerosis, with a mean age of 69.3, using adrenal gland segmentation and cortisol data.
  • Higher Adrenal Volume Index correlated with greater circulating and peak cortisol and allostatic load, and was linked to higher left ventricular mass index; each 1 cm²/m² increase raised heart failure and mortality risk.
  • Because tens of millions of chest CTs are done annually in the United States, the biomarker offers large-scale risk stratification with up to 10-year follow-up, guiding preventive care, researchers said.
