ACA and Medicare in 2027: What’s changing and how to prepare
Insurers cite benchmark shifts, higher out-of-pocket caps and plan exits as some lawfully present immigrants lose subsidy eligibility.
ACA insurers have proposed median premium increases averaging 15% for 2027, with 29 states and Washington having finalized their rates.
Insurers are leaving ACA marketplace plans in more than 20 states, forcing many enrollees to seek new coverage. The marketplace will typically auto-select a replacement plan matching prior coverage.
Certain lawfully present immigrants—including those with student and work visas, refugees, and asylum grantees—will lose federal marketplace subsidy eligibility for 2027. Health policy analyst Louise Norris estimates one million people could become uninsured by 2035.
For 2027, individual marketplace out-of-pocket limits rise to $12,000 from $10,600, while family limits reach $24,000. Medicare Part D out-of-pocket caps increase to $2,400, with payment plans available to spread costs over 12 installments.
Health policy analyst Louise Norris recommends choosing a plan by December 7 to ensure January 1 coverage start. Marketplace open enrollment ends January 15, 2027, though people selecting plans after December 15 face delayed coverage starting in February or March.
