State: Nursing home resident called 911 after being left ‘gasping for air’

The Red Oak Good Samaritan home faces a $27,750 fine after a resident called 911 on April 15, 2026, when staff failed to address her critically low oxygen levels.
Inspectors faulted the home for failing to intervene when the resident showed life-threatening oxygen levels, and for ignoring a separate resident coughing up blood on March 30 and March 31.
Paramedics arriving on April 15 heard audible screams and found the resident 'gasping for air' with oxygen levels in the 'high 60s to low 70s,' while staff allegedly claimed, 'She's been like this all day.'
The Iowa Department of Inspections, Appeals and Licensing proposed the $27,750 fine, which remains suspended while the Centers for Medicare & Medicaid Services determines if a federal penalty is warranted.
Currently holding a one-star CMS rating, the facility failed to offer COVID-19 vaccines to any of its 34 residents in 2025, contributing to broader quality-of-care concerns.
